Хроническая обструктивная болезнь легких и микобактериальная инфекция: особый этиотип и взаимоотягощающая коморбидность

В обзоре литературы суммированы данные о механизмах, клинических особенностях и прогностических последствиях взаимосвязи ХОБЛ и микобактериальных инфекций. Представлены сведения об эволюции концепции хронической обструктивной болезни легких (ХОБЛ), эпидемиологии и клинико-патогенетических особенностях ХОБЛ у пациентов с микобактериальными инфекциями. За последние годы концепция ХОБЛ претерпела существенную трансформацию – от парадигмы болезни курящего человека к полиэтиологическому взгляду на развитие заболевания. Эпидемиологические данные сочетания ХОБЛ и туберкулеза (ТБ) характеризуются существенной вариабельностью. Наличие ХОБЛ у пациентов с туберкулезом (ТБ) и микобактериозом легких (МЛ) приводит к большей длительности бактериовыделения, вероятности развития распространенного поражения легких c деструкцией легочной ткани, а также большей частоте терапевтических неудач. Воспалительный процесс, вызванный микобактериальной инфекцией, способствует ремоделированию дыхательных путей, большей выраженности симптомов и частым обострениям ХОБЛ.
В основе данной патогенетической взаимосвязи лежат общие для ХОБЛ и микобактериальных инфекций факторы риска, морфоструктурные изменения и нарушения иммунного гомеостаза.
